Basic Life and Accidental Death & Dismemberment (AD&D)
Providing economic security for your family if you pass away, become disabled, or experience an injury or illness is a major consideration in personal financial planning. Eligible employees receive Basic Life and Accidental Death and Dismemberment (AD&D) coverage at no cost.

Supplemental Life and AD&D
Voluntary Life and AD&D offer additional coverage for yourself, and/or for your spouse and children. For new hires, the guaranteed issue amount is $250,000 for employees and $50,000 for spouses. For any amount over these limits or outside of the new hire enrollment period, Evidence of Insurability forms must be completed.
Employee Assistance Program (EAP)
As part of your Basic Life and AD&D coverage, you have access to Ability Assist® Counseling Services, an Employee Assistance Program (EAP). When you need a helping hand, the EAP offers value-added programs and services for free. This confidential service is available to you and your family members to help with many life challenges including grief, stress, depression, marital or family challenges, retirement planning, alcohol and drug abuse, and more.
The EAP gives you access to:
- Unlimited 24/7 phone counseling sessions
- In-person counseling sessions
- Child and elder care resources
- Financial and legal resources
- And more
You can contact the EAP anytime, day or night, by calling 800-964-3577 or by visiting www.guidanceresources.com. If you visit the website as a first-time user, click on the Register tab and follow the steps below to create a new account:
- In the Organization Web ID field, enter: HLF902.
- In the Company Name field at the bottom of the personalization page, enter: ABILI.
- After selecting Ability Assist program, create your own confidential username and password.

Voluntary Accident
Since accidents can happen at any time, it’s important to prepare for the unexpected. Accident insurance can help pay for out-of-pocket expenses associated with an accident by paying you a benefit for each of the covered injuries you suffer and the treatment you received. This policy does not coordinate with any other coverage, so you can still receive benefits on top of what your medical plan provides.
Voluntary Critical Illness
Critical illness insurance protects your family when you are diagnosed with an unexpected, covered condition by providing you with a lump sum cash benefit in the event you or an insured family member is diagnosed with a covered condition. This benefit can be used to cover medical expenses, lost income, or other financial burdens, offering peace of mind and financial support during a challenging time. This policy does not coordinate with any other coverage, so you can still receive benefits on top of what your medical plan provides.
Voluntary Hospital Indemnity
Hospital Indemnity insurance protects your family when you have a hospital or ICU stay. This policy provides financial protection by paying you a benefit for hospital admission, hospital confinement and ICU care allowing you to focus on your recovery rather than worrying about unexpected expenses. Benefits are paid based on admission and length of stay for a defined number of days. This policy does not coordinate with any other coverage, so you can still receive benefits on top of what your medical plan provides.
